Urticaria y/o angioedema crónico y su relación con reacciones adversas a alimentos

La urticaria y el angioedema crónico (UAC) son trastornos cutáneos con características similares, la etiología en la mayoría de los pacientes es de difícil diagnóstico, sin embargo la historia clínica médica y familiar es un instrumento dignóstico muy útil, ya que permite identificar factores desencadenantes. Con el objetivo de determinar la relación de la urticaria y/o angioedema crónico con las reacciones adversas a los alimentos, en niños de 1 a 15 años que acuden a la consulta externa de alergia del Hospital del Niño “Dr. Rodolfo Nieto Padrón,” se realizó un estudio prospectivo, longitudinal y descriptivo del 15 de enero al 15 de diciembre de 1998. Se estudió un total de 34 pacientes en los que se investigaron los siguientes datos clínicos y de laboratorio: edad, sexo, domicilio, diagnósticos asociados, relación causal, atopia familiar, exámenes de laboratorio, pruebas cutáneas, tratamiento establecido y evolución clínica. De acuerdo al objetivo del estudio, la relación causal con la ingesta de alimentos se dio en 16 casos (47%) y mediante la prueba cutánea a alimentos sólo en 6 casos (17.6%) resultaron positivas. En este estudio, la intolerancia o alergia a los alimentos se consideran como factor causal importante en el desarrollo de UAC, así como el antecedente de atopia familiar.

The urticarya and chronic angioedema (CAU) are cutaneous diseases with similar characteristics, the etiologies in most of the patients is difficult diagnostic, however medical clinical history and familiar it is a very useful diagnostic instrument, since it allows to identify leading factors. With the objective of determining the relation ship of the chronical angioedema the urticary urticaria and/or angioedema with the adverse reactions to foods, in children of 1 to 15 years who attend the external consultation of allergy of the Children Hospital Dr. Rodolfo Nieto Padrón, a prospective, longitudinal and descriptive study was made from 15th January to the december 15th of 1998. A total of 34 patients in whom the following clinical data were investigated and of laboratory studied: associated age, sex, address, diagnostics, familiar causal relation, atopia, tests of laboratory, cutaneous tests, established processing and clinical evolution. According to the objective of the study,the causal relation with the food ingestion occurred in 16 cases (47%) and by means of the cutaneous test to foods only in 6 cases (17.6%) they were positive. In this study, the intolerance or allergy to foods is considered like important causal factor in the development of CAU, as well as the familiar antecedent of atopia.
